I have been informed, that practicing these activities can be dangerous to health and life, even if practiced correctly with the observance of safety rules and proper belaying. As a participant in these activities I am aware of and accept the risk and therefore I am responsible for my own actions and involvement.

I declare that I have no medical contraindications to participate in the course. My health allows me to practice climbing and mountaineering. I undertake to inform the instructor about any diseases, medication, psychophysical indisposition or other restrictions.
